Job description
JOB DESCRIPTION

Serves as a lead Pharmacotherapy Specialized Healthcare Professional by efficiently implementing and leading the designing, recommending, activating, monitoring, and evaluating patient-specific and protocol driven medication management to ensure optimal outcomes. Ensures implementation of the pharmaco therapeutic care plan, monitors for achieved outcomes and resolution and places measures to manage the Drug Related Problems (DRPs). Maintains liaison relations with medical and nursing staff. Plays a fundamental role in the development of clinical pharmacy practice through establishment of new Policies and Clinical Practice Guidelines, specialized service, and conducting clinical audits and research.

RESPONSIBILITIES
  • Optimizes patient care in defined units by planning, managing, monitoring and advising specialist pharmaceutical care programs for patients.
  • Identifies and undertakes a medication review/ history of newly admitted patients to resolve issues that may influence the patient's treatment plan.
  • Formulates treatment plans for inpatients, considering their presenting symptoms, medical history, and the evidence base for pharmacological approaches in that condition.
  • Conducts clinical evaluations to assess medication use appropriateness.
  • Creates future treatment options with patients/caregivers after considering and discussing the risks and benefits.
  • Assumes responsibility for patient-specific pharmacotherapy plan to ensure optimal outcomes.
  • Monitors medication orders for drug interactions, duplication and omission of therapy, allergies, adverse reactions, and recommends appropriate formulary alternatives as needed.
  • Advises on pharmaceutical dosage (pediatric doses, IV admixtures, TPN, chemotherapy, etc.).
  • Liaises with physicians, nurses and other fellow health care professionals to ensure the delivery of safe, effective and economic drug treatment.
  • Interprets laboratory tests and other diagnostic tools to assess the safety and effectiveness of drug therapy for individual patients, and recommend changes to practice and policy in line with updated evidence based medicine.
  • Maintains documents related to therapeutic recommendations in the patient's electronic medical record using the relevant pharmacy intervention tools.
  • Oversees the safe and competent use of all pharmacy equipment used in their area of practice.
  • Provides regular attendance at multidisciplinary team meetings and medication review sessions making clinical decisions, and is able to support these when challenged with a logical rationale and evidence base.
  • Participates in department programs/committees, various hospital committees and staff meetings, as assigned.
  • Facilitates the orientation and training of new employees.
  • Provides drug information and education resource for patients, students, colleagues, medical and nursing staff, and other healthcare professionals.
  • Contributes to advancing the professional knowledge by participating in scientific committees of conferences, seminars and other events.

Role-Specific Job Responsibilities
  • Assumes leadership role in area of specialty that drives implementing advanced clinical pharmacy practice, contribution to new services and enhancing the safety of the HIS.
  • Leads formulary management initiatives that include evidence based, efficacy, safety and cost-effectiveness driven reviews for medication addition/deletion.
  • Leads the Pharmacy and Therapeutics, PTC discussions that include review of medication safety reports, ASP and other subcommittees of the PTC. Oversees medication orders and patient medication profiles for appropriateness of therapy.
  • Supervises team of Clinical and Unit Based Pharmacists as assigned by Pharmacy Manager. Ensures specialized Clinical Pharmacy service across the organization.
  • Implements development and succession plans that sustain continuity of specialized Clinical Pharmacy Services.
  • Leads and supervises advanced education and training programs including Pharmacy Residency and postgraduate rotations.
  • Supports and supervises research activities and ensures compliance with ethics and principles of well conduct.
  • Contributes to development of the profession through participation in national and international relevant Clinical Pharmacy organizations.
QUALIFICATIONS

Experience:
Required:
  • NLT 6 years as a hospital pharmacist of which 5 as clinical pharmacists or residency training programs providing Pharmaceutical care.
  • UAE National: NLT 3 years as a hospital pharmacist of which 2 as a clinical pharmacist or with clinical responsibilities providing Pharmaceutical care.
Desired:
  • Evidence of teaching/mentoring other pharmacists.
  • Evidence of research activity.
Educational Qualification:
Required:
  • BSc Plus MSc. Or Doctor of pharmacy (Pharm. D) degree from an accredited college of pharmacy.
  • AND Current National License.
Desired:
  • Board Certification or (residency in clinical pharmacy).
