Key Responsibilities:
- Perform thorough evaluations of geriatric patients, taking into account their medical, social, and psychological needs.
- Design and implement personalized care plans, ensuring that they are tailored to each patient's unique situation.
- Monitor and manage chronic conditions, including but not limited to diabetes, hypertension, and cognitive impairments.
- Educate patients and their families about managing health conditions, preventive care, and promoting healthy lifestyles.
- Collaborate closely with a multidisciplinary team to provide comprehensive care that addresses all aspects of a patient's health.
- Maintain accurate medical records and documentation to comply with regulatory requirements.
- Stay informed about the latest research and advancements in geriatric medicine through continuous education.
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification in Geriatric Medicine is mandatory.
- Possession of a valid medical license in the relevant state.
- A minimum of 3-5 years of experience in a geriatric or relevant medical setting.
- Comprehensive knowledge of age-related diseases and treatments, along with a holistic approach to patient care.
