Job Description
Assists patients, families, and significant others with their psychological, social, environmental, financial, and emotional problems by conducting the needed psychosocial assessments related to presenting problems/concerns, providing the necessary interventions to enhance patient quality of living.
Responsibilities
- Managerial
- Involved in daily management of workload allocation within the specialist area, considering changing demands and prioritizing caseloads.
- Potential supervision of junior staff, ensuring adherence to rules and policies.
- Technical
- Manage care for vulnerable clients, including comprehensive assessments, social care planning, and resource identification.
- Coordinate delivery of social care plans and liaise with government or charity agencies.
- Conduct initial psychosocial assessments, develop care strategies, and update patient records as per JCI standards.
- Advocate for patients, ensuring their wishes are central in care planning.
- Address legal issues, liaising with authorities and providing necessary reports for safe discharge.
- Participate in safe decision-making for discharge planning.
- Collaborate with agencies and professionals to provide comprehensive care.
- Guide patients and families to appropriate resources and support.
- Follow up on psychiatric visits, document conditions, and maintain records.
- Respect patient rights, spiritual beliefs, and customs.
- Follow up on ER exempted cases, ensuring treatment coverage.
- Promote awareness of social work and anti-discriminatory practices.
- Network with professionals and organizations for service provision and resource access.
- Empower individuals and families, advocating for their rights and informed decisions.
- Maintain high standards of record-keeping and professional conduct.
- Manage exposure to distressing situations, maintaining mental resilience.
- Ensure confidentiality and compliance with legislation.
- Provide crisis interventions as needed.
- Participate in discharge planning and multidisciplinary team meetings.
- Deliver psychoeducation and therapeutic interventions as appropriate.
- Quality & Safety
- Ensure compliance with safety and quality procedures.
- Maintain infection control and environmental safety standards.
- Ensure confidentiality of patient information.
- Comply with occupational health and safety policies.
- Assist in policy development and ensure SOP and QA standards are met.
- Participate in training, seminars, and professional development.
- Complete all regulatory requirements, including licensure and certifications.
- Engage in clinical supervision for professional growth.
- Education
- Participate in training sessions, department committees, and staff meetings.
- Assist in onboarding new staff.
- Share information appropriately, maintaining confidentiality.
- Maintain ongoing professional development through education and seminars.
Qualifications
- BSc in Social Work and Master’s degree in Social Work
- Required: Two years of experience
