Qualifications

• Previous experience in home healthcare or clinical/hospital settings.
• DHA license or eligibility is preferred.
• Ability to travel within assigned areas as required.

Responsibilities

• Provide personal care and basic medical assistance to patients in their homes.
• Monitor, record and report changes in patients' health conditions including vital signs.
• Accompany patients to medical appointments or errands as required.
• Maintain accurate and up-to-date care documentation and daily reports.
• Previous experience in home healthcare or a clinical/hospital setting.
• DHA license or eligibility is preferred.
• Responsible, compassionate, and adaptable to varying patient needs.
• Willingness to travel within assigned areas as required.
