Job description

The Clinical Coder reviews, analyzes, and codes documentation for hospital medical records to select and sequence the appropriate ICD-10-CM diagnosis and CPT procedure codes as applicable. This position is responsible for accurate assignment, abstracting to determine accuracy and completeness of the record, and utilizing the 3M Coding Reimbursement system to compile data and complete assigned work. Coding staff in this position are capable of coding encounters across multiple specialties, as well as mentoring and training other coding levels. The role involves using encoder software, online tools, and references to facilitate code assignment, consulting reference materials as needed. The Clinical Coder validates charges by comparing them with health record documentation, understands the appropriate linkage of diagnosis to procedure when applicable, and uses retrospective edit tools to address potential coding or documentation issues. They also consults with CDI, physicians, or other healthcare providers for additional information or clarification, and collaborates with Revenue Cycle Management teams to resolve billing and utilization issues impacting reimbursement.

Required:
2-4 years of relevant progressive experience in a similar role

Educational Qualification:
Required:
Bachelor Degree in Health Information Management or relevant field with or Diploma with 3 years of additional healthcare experience

Desired:
Master degree or equivalent in Health Information Management or relevant field

Additional Responsibilities include assigning codes for diagnoses, treatments, and procedures for complex inpatient, outpatient, emergency, or observation encounters; reviewing provider documentation; adhering to coding guidelines; clarifying diagnosis and procedural information; assigning POA values; interacting with physicians; verifying patient disposition; maintaining productivity and accuracy; prioritizing work; engaging in coding query processes; adhering to ethical coding standards; preparing statistics; and supporting team collaboration and a positive work environment.

Accountabilities include ensuring accurate and compliant coding, implementing strategic procedures, and evaluating methods to ensure correct results.
