Key Purpose

The role involves acting as a self-directed practitioner providing and evaluating evidence-based care to patients at home, based on management care plans and protocols. Responsibilities include performing assessments, delivering interventions such as cannulation, IV antibiotics, fluids, medications, urinary catheterization, palliative care, and wound care (training can be provided). The role also ensures robust patient onboarding and effective communication with stakeholders to ensure safe and quality care.

Key Outputs

The successful candidate will be responsible for:

  • Performing holistic, patient-centered assessments and formulating shared management plans.
  • Communicating effectively with medical teams.
  • Providing high-quality interventions including intravenous therapies and wound management.
  • Recognizing emergency situations and acting appropriately.
  • Assessing home safety and liaising with multidisciplinary teams to meet patient needs.
  • Maintaining accurate records in line with POPIA.
  • Onboarding patients and managing remote monitoring devices.
  • Building relationships with device vendors and troubleshooting issues.
  • Providing education and information to patients and families.
  • Facilitating medication collection and treatments.
  • Adapting to changing circumstances and making effective clinical decisions.
  • Providing on-call and after-hours support.
  • Supporting team development and service improvement.

Experience, Qualifications, and Mandatory Requirements

  • Bachelor's Degree / Diploma in Nursing
  • Registered with SANC and practicing
  • At least 3 years' experience in community or emergency/urgent care settings
  • Proficiency in Microsoft Office and computer literacy
  • Basic Life Support certification
  • Valid registration with SANC or HPCSA
  • Full COVID-19 vaccination
  • Own transport and valid driver's license

Advantageous Skills

  • Specialization in IV therapy, wound care, and pain management
  • Experience in home-based care
  • Exposure to therapeutic disciplines
