Job description

Support Coordinator

Pay £13.21 per hour (£25,000 per annum) and great benefits including a Health Cash Plan! Home: a place where you belong. Want to be part of a team that cares and empowers you to help change customers' lives for the better?

As our Support Coordinator, you'll play a crucial role in supporting the smooth running of our service.

You'll be working with young people aged between 16-25.

We assist them with emotional support, building independent living skills, providing advice, and advocating for our customers.

Many have mental health diagnoses and may present challenging behaviors.

You will make a significant difference each day by helping our customers achieve their hopes and aspirations.

Typical Day as a Lead Support Worker (Coordinator)
  1. Creating person-centred support plans with customers, working with clinicians to develop Risk Management Plans.
  2. Assisting customers with complex mental health needs.
  3. Helping with daily activities such as meal preparation, shopping, budgeting, maintaining tenancy, and moving to independent accommodation if able.
  4. Carrying out risk assessments, support planning, goal setting, and reviews.
  5. Working closely with external agencies for seamless support.
  6. Supporting support workers in service delivery, focusing on KPIs, and collaborating with management.
Requirements
  • Passion for supporting customers in living their best lives with attention to detail.
  • Experience in creating person-centred support plans and supporting colleagues.
  • Experience in coordinating and assessing customer referrals.
  • Ability to work independently, remain calm under pressure, and be resilient.
  • Experience in safeguarding, risk assessments, confidentiality, and data protection.
  • Technologically savvy (IT skills).

  • Work on a rota with various shifts, including weekends and bank holidays.
  • Lone working involved, with regular team catch-ups at Simonside House.
  • Use technology for support plans, online learning, and collaboration.
  • Enhanced DBS check required (we pay for this).
