Overview

Function Rehabilitation Facility Position Social Worker | Permanent Introduction

A position has become available for a Permanent Social Worker, based at Life St. Dominic’s Rehabilitation Unit, reporting to Carina Basset, Therapy Services Coordinator. Life Rehabilitation is part of an expanding network of rehabilitation units within Life Healthcare Group.

Critical Outputs
  • Evaluation, programme planning and treatment of patients admitted to the unit
  • Education of patients and their families about patient’s condition
  • Setting of common psychosocial goals with patients and the team
  • Co-ordination of family meetings and feedback to families and employees
  • Co-ordination of counselling programs
  • Co-ordination of discharge planning including placement if required
  • Co-ordination of return work planning in conjunction with the Occupational therapist
  • Active participation in continuous professional development activities of the unit
  • Building relationships with referring doctors and therapists
Requirements
  • Applicants need to have a degree in Social Work with a special interest in neurological rehabilitation
  • Experience in neurological related counselling and discharge planning would be an advantage
  • Ability to work in a team environment is essential
  • The candidate will need to demonstrate commitment to quality patient care and customer satisfaction
  • Able to work some weekends and public holidays
