Qualifications

• Bachelor's degree in Social Work, Psychology, Occupational Therapy or related field.
• Experience in case management or coordination within rehabilitation.
• Strong understanding of rehabilitation methodologies and recovery principles.
• Excellent organizational, communication, and interpersonal skills.
• Ability to work with diverse client populations and multidisciplinary teams.
• Knowledge of Mahikeng region support services.

Responsibilities

• Assess clients' needs and develop comprehensive, individualized rehabilitation plans.
• Coordinate services with healthcare providers, therapists, social workers, and other support professionals.
• Monitor client progress, adjust treatment plans as necessary, and provide ongoing support.
