Job description

Facilitation and support of the reporting and investigation of Adverse Events Following Immunisation (AEFI) reporting and investigation in the province and district.

  1. Alert the National Department of Health, Expanded Programme on Immunisation AEFI Coordinator of serious/severe AEFIs.
  2. Assign EPID numbers for reported AEFI cases in consultation with district public health officials.
  3. Assist with follow-up of cases and ensure submission of all Case Report Forms (CRF), Case Investigation Forms (CIF), supporting documentation (clinical notes, laboratory results, autopsy, verbal, post-mortem, etc.), and line lists to the National AEFI Coordinator and their capture on the electronic system (MedSafety-Vigilance Hub).
  4. Monitor the timely submission of CRF and CIF, and investigate severe or serious cases, clusters, and adverse events of special interest (AESI).
  5. The provincial vaccine safety surveillance team follows up to ensure all documentation is submitted and captured on the electronic system.
  6. Serve as a Vigilance Hub super-user, responsible for generating AEFI reports, line lists, and assigning cases to district public health officials for follow-up.
  7. Train AEFI Coordinators on the use of the Med Safety App and Vigilance Hub for capturing AEFIs, reporting, adding missing information, uploading supporting documents, drawing line lists, and generating reports.
  8. Capture paper-based CRFs on the Vigilance Hub, update cases as investigations proceed, and upload all related investigation forms, clinical records, and autopsy results.
  9. Prepare formal communications and updates/reports on AEFI surveillance.
  10. Assist the provincial team in analyzing CRFs and guiding investigations.
  11. Serve as secretary for the Provincial review and investigation team meetings.
  12. Attend National and Provincial AEFI Team meetings.
  13. In case of death within 30 days of vaccination, alert the provincial AEFI review committee and the National Department of Health for further guidance, submitting CRF, CIF, clinical records, and if applicable, the Med Safety App reference number.
  14. Ensure necessary post-mortem examinations are completed in cases of death within 30 days after vaccination.
