Job summary

Highway Hospice in Durban seeks a Professional Nurse to join the Home-Based Care team, delivering expert palliative care in patients' homes and supporting families.

Applicants must be a registered nurse with SA Nursing Council, have at least five years of experience, and hold a valid manual driver's license. Palliative care experience is an advantage.

Qualifications

• SA Nursing Council registration is required.
• Minimum 5 years of experience as a Registered Nurse.

Responsibilities

• Deliver high-quality palliative care to home-based patients and manage pain and symptoms.
• Provide psychosocial support to patients, families and caregivers.
• Develop and promote evidence-based palliative nursing care protocols in HBC.
• Formulate, implement, and evaluate individualized care plans for patients and caregivers.
• Maintain accurate patient records and medico-legal documentation.
• Handle referrals in collaboration with Admissions office.
• Demonstrate leadership to promote teamwork and compassionate care.
• Communicate clearly with staff, patients, families and caregivers.
• Manage resources within the Home Care Department and Highway Hospice.
• Collaborate with the interdisciplinary team to deliver person-centered care.

About Highway Hospice

Highway Hospice is a registered non-profit organisation that has served Durban for over forty years, providing palliative and end-of-life care to patients with life-limiting illness and their families. Our 24-hour In-Patient Unit (IPU) will offer short-stay symptom control, respite and end-of-life care alongside our established Home-based service.

The Role/Key Responsibilities

We are looking for an experienced and mature Professional Nurse to join our Home Care Department, to provide palliative care to our home-based patients.

• To deliver high quality comprehensive palliative care to our home-based patients, diagnosed with a terminal illness, and their caregivers in their own homes by way of managing pain and symptoms.
• Provide psychosocial support to patients, their families and caregivers
• Develop and promote evidence-based palliative nursing care protocols across our HBC
• Formulate, implement, and evaluate individualized care plans for each patients and their caregivers.
• Maintain accurate patient records, documentation and medico-legal reports
• Handle, maintain and manage new patient referrals whilst working alongside Admissions office
• Demonstrate leadership skills to promote effective teamwork and caring for caregivers
• Demonstrate good communication skills, written and verbal, with staff patients, family members and caregivers
• Effective management of resources, human, technical and financial within the
• Home Care Department and Highway Hospice
• Collaborate with all members of the interdisciplinary team to deliver person centred care

Requirements & Qualifications

To be considered for this role, you must meet the following criteria:

• South African Nursing Council Registration
• Minimum of 5 years working experience as a Registered Nurse
