We are seeking a highly motivated, innovative, and enthusiastic Band 5 Primary Care Staff Nurse to join our friendly and expanding Health & Justice team at HMP Coldingley. Main duties of the job

Staff within Primary Care will:

  • Provide accessible individual patient assessments, treatments, and referrals.
  • Monitor patients' health status and address issues.
  • Work with patients to influence health outcomes related to lifestyle choices.
  • Implement effective discharge plans for transfer or release.
  • Coordinate multidisciplinary clinical practices for integrated care.
  • Collaborate with staff from other disciplines to provide coordinated treatment plans.

Key Tasks:

  • Triage patient needs following SOPs and national policies (e.g., NICE guidance).
  • Assess, plan, and deliver care within treatment pathways to promote wellbeing.
  • Use evidence-based practices to ensure effective clinical care.
  • Support the development of integrated healthcare services.
  • Act as a preceptor/mentor based on experience.

  • Assess, plan, implement, and evaluate patient care using PGDs or care pathways.
  • Manage workload independently to meet site requirements.
  • Administer prescribed medications and treatments, managing interruptions effectively.
  • Refer patients requiring specialist input promptly and document appropriately.
  • Complete assessments for at-risk patients and communicate concerns.
  • Provide health promotion interventions and participate in clinics (e.g., smoking cessation, immunisations, sexual health).
  • Transport and collect specimens as needed.
  • Perform wound dressings and ensure proper discharge planning.
  • Participate in emergency responses and case reviews.
  • Undertake other duties as required, including cover for other teams or sites.
