Job Summary

The position reports to the Facility Manager and focuses on providing outpatient rehabilitation services through a collaborative care model with physicians to optimize patient performance and function.

Responsibilities
  1. Provide exceptional patient care
  2. Interpret physician orders, perform evaluations, establish treatment plans and goals
  3. Adjust treatment goals as needed
  4. Document patient status and progress
  5. Determine discharge and follow-up timelines
  6. Supervise Rehabilitation Technicians and staff as needed
  7. Maintain timely documentation and ethical billing practices
  8. Maintain active license and pursue CEUs
  9. Collaborate with physician teams and participate in programs
Requirements
  • Graduate from an accredited Physical Therapy Program
  • Licensed or eligible in Illinois
  • Ability to work in a fast-paced environment
  • Strong communication skills
  • Ability to lift >25 pounds and understand proper body mechanics
  • Ability to stand and sit for extended periods
  • Proficiency in computer use
