Description: Responsible for providing medically prescribed physical therapy to patients in the home in accordance with agency policies. Participates in developing and implementing the plan of care with goals and documentation appropriate to discipline guidelines. Provides supervision as needed to PTA per JCAHO / CHAP, Florida State and Federal practice act guidelines.
Qualifications:
- Bachelor's degree and graduation from an accredited school of Physical Therapy.
- At least one year of work experience as a Physical Therapist.
- Ability to prioritize, manage time effectively, and make independent decisions.
- Current Florida Physical Therapist License.
- CPR certification.
- Valid driver's license with access to a vehicle and insurance coverage.
Job Relations: Reports to the Director of Clinical Services.
Main Duties & Responsibilities:
- Performs physical therapy procedures, evaluations, resumption of care, admissions, and diagnostic tests as prescribed, establishing measurable goals.
- Teaches treatment procedures and supportive activities to patients and families.
- Translates exercises into functional activities or activities of daily living.
- Develops plans for modifying equipment, appliances, and the physical environment in the patient's home.
- Explains treatment implications to patients and families, aligning with physician's directives.
- Helps families accept responsibility for care, providing support and encouragement.
- Assists individuals and families in adapting to physical, mental, and social limitations, identifying barriers to goals.
- Evaluates patient progress, adjusting therapy as needed under physician guidance.
- Teaches principles of good posture and body mechanics across age groups.
- Maintains necessary records.
- Demonstrates procedures to nurses responsible for follow-up.
- Supervises home health aides when appropriate.
- Participates in recertification and interdisciplinary team meetings.
- Completes OASIS assessments accurately and timely.
- Notes progress towards discharge, communicates with physicians and team members.
- Participates in quality improvement activities and agency processes.
- Discusses job performance and areas for improvement.
- Flexible with workflow, department needs, and work hours.
- Updates management on potential issues and consults with clinicians or managers as needed.
- Maintains understanding of company scope and infrastructure, ensuring proper call transfers.
- Maintains professionalism, promotes a customer-friendly environment, and ensures patient confidentiality.
- Participates in accreditation programs and performs other duties as assigned.
