Are you a dynamic and compassionate healthcare professional with a passion for helping patients regain their independence? Join our interdisciplinary team as a Case Manager in our Physical Rehabilitation Unit and play a key role in coordinating patient care and optimizing clinical outcomes.

About The Role

The Clinical Case Manager is responsible for facilitating the patient’s rehabilitation journey from pre-admission to discharge, ensuring a coordinated, patient-centered approach to care. This role requires close collaboration with the medical, nursing, therapy, and social work teams to support safe transitions, maximize functional recovery, and ensure efficient use of healthcare resources.

Key Responsibilities

  • Coordinate and manage the full case management process for patients in the rehabilitation unit.
  • Monitor patient progress and adjust care plans in collaboration with the interdisciplinary team.
  • Liaise with patients, families, funders, and referring physicians to ensure continuity of care.
  • Ensure accurate documentation and maintain communication with all stakeholders.
  • Facilitate timely discharge planning and referrals to appropriate community-based services.

Requirements

  • Registered Nurse (SANC) or equivalent Allied Health qualification (e.g., Physiotherapist, Occupational Therapist, Social Worker).
  • Minimum of 3 years’ clinical experience, preferably in rehabilitation, acute care, or case management.
  • Proven knowledge of rehabilitation processes, funding models, and discharge planning.
  • Strong interpersonal, communication, and coordination skills.
  • Ability to work effectively in a interdisciplinary team environment.

REMUNERATION
