Rota :

Monday to Friday 9-5 with a late shift on Thursdays (2pm-10pm)

Position Overview

We are seeking an Outreach Worker to join our East London-based client's outreach team in Hackney. This role involves supporting the rough sleeper community in Central London.

This is a very important position.

Key Responsibilities

1. Advise street populations on available options and safety measures.
2. Listen to concerns and provide support.
3. Manage a caseload of 10-15 rough sleepers and collaborate with the team.
4. Work in pairs or alone with risk assessments to attend appointments.
5. Assist rough sleepers in accessing social care and accommodation.
6. Respond to referrals with needs and risk assessments.
7. Update assessments and the CHAIN database regularly.
8. Determine appropriate actions based on individual scenarios.
9. Collaborate with agencies including local authorities, health services, housing providers, police, and substance misuse services.

Qualifications and Skills

1. Excellent interpersonal skills.
2. Flexibility and responsiveness to client needs.
3. Ability to manage complex caseloads confidently.
4. Proficiency in IT for data collection and analysis.
5. Ability to represent the organization at external meetings.
