National Promotions Admin Controller

We are looking to add to our Prairieland family in Decatur, IL.

Prairieland is seeking a Case Manager / QIDP to serve Effingham, Coles and Shelby Counties.

Qualifications:

1. Bachelor's degree in human service related field.
2. One-year cumulative experience providing direct service to individuals with developmental disabilities required.
3. Experience can be through employment, volunteer, or family care.

Schedule: 8:00 am to 4:30 pm, Monday through Friday, with flexibility to work partially from home.

General Description: Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc., the Case Manager assesses individual needs and assures the provision of necessary services through communication, service coordination, linkage with provider agencies, service monitoring, and individual advocacy.

Job Responsibilities:

1. Completes the PAS process, following the PAS manual procedures.
2. Advocates on behalf of the individual and/or family to assist in obtaining eligible services.
3. Ensures the involvement of the individual and their family in the process.
4. Reviews individual’s medical, residential, vocational, educational, legal, and financial status needs through Visiting Notes.
5. Schedules and completes Discovery and Personal Plan process annually or as required.
6. Maintains individual files as specified by program and agency guidelines.
7. Provides problem-solving consultation with provider agencies following the DHS Conflict Resolution guideline.
8. Coordinates specialized services for eligible Bogard individuals and submits summary reports to external agencies.
9. Monitors services provided to Bogard class members on a monthly basis and advocates for any unmet needs.
10. Enters billings daily into Birdseye to ensure timely payroll reporting.
11. Participates in scheduled all staff meetings and mandated training sessions.
12. Alerts the Program Manager of PSCI to problematic issues, trends, or service gaps.
13. Performs other related work as required or requested by the Executive Director and/or Program Manager.

Job Qualifications:

1. A bachelor's degree in human services required and QIDP eligibility as established by Department of Human Services.
2. Ability to organize, plan, and follow a schedule.
3. Working knowledge of the developmental disabilities service delivery system.
4. Ability to work effectively with professionals, social service organizations, and clients.
5. Ability to work well in teams and independently with self-discipline.
6. Flexibility to work hours outside of the normal workweek.
7. Willingness to follow directions and accept constructive criticism.
8. Reliable transportation to work and maintain a valid Illinois Driver's License and personal auto insurance.

Physical Expectations:

1. Must be able to read, write, and communicate effectively.
2. Must be able to ambulate, sit, and bend.
3. Must be able to carry files and lift up to 5 lbs.

Hourly Wage Range: $19.95 - $21.63
