Medical Social Consultant (Home Care Enrollment Specialist)

Location: Chicago, IL, USA

Department: Division of Specialized Care for Children

Schedule: 8:00 am - 4:30 pm, Days, Hybrid work environment

Salary Range: $52,000 - $63,000 annually, commensurate with experience

Position Summary: The Home Care Enrollment Specialist provides services to families potentially eligible for DSCC Home Care Services, coordinating and facilitating the discharge process from inpatient hospital settings and in-home nursing services.

Key Responsibilities:

  1. Assist families with the application process for in-home nursing and related services.
  2. Gather documentation to determine eligibility for DSCC programs and explain enrollment procedures.
  3. Conduct home assessments to ensure safety and determine need for modifications.
  4. Coordinate healthcare benefits and assist families in interviewing and monitoring home care providers.
  5. Collaborate with hospital staff and other providers to ensure timely service delivery at discharge.
  6. Perform health assessments and ensure data entry compliance.
  7. Participate in multidisciplinary team meetings and agency committees.
  8. Promote interagency collaboration and community education about DSCC services.

Minimum Qualifications:

  1. Master's degree in social or behavioral sciences or related health field, or
  2. Three years of responsible social work experience in a medical or social service setting.
  3. Current State of Illinois licensure as a Licensed Social Worker or Licensed Clinical Social Worker, or eligibility to obtain such licensure.
