Qualifications

  • Minimum one year nursing experience, preferably in hospice or home health.
  • CA Licensed Practical/Vocational Nurse license and CPR certification required.

Responsibilities

  • Provides direct patient care, including specialized duties related to end of life care.
  • Work collaboratively with the interdisciplinary team to meet patient care outcomes.
  • Participate in after hours on-call, holiday, and weekend rotations.

Job description

Join to apply for the LVN role at Seva Hospice. Seva Hospice is a locally owned and operated company of experienced hospice providers with a desire to serve the communities we live and work in.

Due to growth, we are looking for an experienced Licensed Vocational Nurse to join our dynamic team serving Tuolumne, Calaveras and Amador counties.

This is a part-time after hours, on-call position from 12:00 am – 8:00 am. Your presence and services should bring comfort to the person who faces the end of their lives and help them reach acceptance and peace of mind.

A compassionate nature and excellent interpersonal skills will make you a good candidate for this position.

Our goal is to help patients live their final days with dignity and comfort.

Skills

Responsibilities
  • Provides direct patient care, including specialized duties related to end of life care.
  • Follow the plan of care and provide treatments, support, and comfort according to individual patient's needs.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
  • Work collaboratively with the members of the interdisciplinary team to help meet positive patient care outcomes.
  • Alleviate symptoms to the patient's desired level of comfort and help in administering medication.
  • Evaluates patient's response to treatments/medications.
  • Participates in personal and professional growth and development.
  • Also participates in organization's quality assessment performance improvement teams and activities.
  • Notify a doctor for a patient's condition when deemed necessary.
  • Assist patients and their families with paperwork.
  • Maintain accurate records.
  • Help relatives cope with the upcoming loss and provide post-mortem support.
  • Participate in after hours on-call, holiday, and weekend rotations.
