Placements24 seeks a dedicated Family Support Practitioner to assist families in Mossel Bay, Western Cape. The role involves in-home and community-based interventions, assessing family needs, and developing tailored support plans with a focus on child well-being.

You will coordinate with schools and social services, provide crisis intervention, and maintain confidential records while promoting resilience and positive family dynamics.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Minimum of 3 years of experience in family support, child protection, or community services.
• Demonstrated experience in conducting family assessments and developing intervention plans.
• Knowledge of child development, family dynamics, and common social challenges.

Responsibilities

• Provide direct support and intervention to families in homes and community.
• Conduct comprehensive family assessments to identify needs, strengths, and risks.
• Develop and implement individualized family support plans with families.
• Facilitate access to community resources (counseling, housing, education).
• Counsel on parenting skills, child development, and family communication.
• Collaborate with schools, social services and agencies for coordinated support.
• Provide crisis intervention and support to families in distress.
• Maintain confidential case notes and program documentation.
• Monitor family progress and evaluate interventions.
