Qualifications

• Bachelor's degree in Social Work, Psychology, Disability Studies, or related field.
• At least 4 years' experience in disability support or case management.
• Knowledge of South Africa disability legislation and support frameworks.
• Proven ability to coordinate services and manage multiple client cases.
• Excellent interpersonal, communication and problem-solving skills.
• Comfortable with hybrid work, including remote client engagement.

Responsibilities

• Assess client needs and develop individualized support plans.
• Coordinate access to therapy, personal care, assistive tech, education programs.
• Liaise with government agencies, healthcare providers, and community orgs.
• Provide ongoing support and advocacy for clients' rights and needs.
• Facilitate communication between clients, families and providers via in-person and virtual methods.
• Maintain accurate case notes, progress reports, and admin records.
