Health Coaches offers a service through a set of health and lifestyle programmes designed to facilitate self-management and is offered to covered persons based on certain eligibility criteria.
We promote sustainable lifestyle change and support through behavioural change, emotional support, benefit navigation, goal setting, and tracking to decrease the cost of health care and improve a member's quality of life.
Coaching eligible members by making use of interpersonal communication and related techniques designed to engage clients actively and safely in health behavior change about evidence-based clinical strategies and interventions to better self-manage their health, health risks, and acute or chronic health conditions resulting in optimal wellness, improved health outcomes, lowered health risk, and decreased health care costs.
