Job summary

An established industry player is seeking passionate individuals to join their Health Coaches team. This role involves coaching members to facilitate sustainable lifestyle changes, improve health outcomes, and manage health risks effectively. You will utilize evidence-based strategies and interpersonal communication to engage clients actively in their health journey. Qualifications

  • Clinical qualification and registration with health professions body required.
  • At least two years of work experience in the health profession.

Responsibilities

  • Coach members on health behavior change using communication techniques.

  • Record and track interactions and outcomes throughout coaching.

About Health Coaches

Health Coaches offers a service through a set of health and lifestyle programmes designed to facilitate self-management and is offered to covered persons based on certain eligibility criteria.

We promote sustainable lifestyle change and support through behavioural change, emotional support, benefit navigation, goal setting, and tracking to decrease the cost of health care and improve a member's quality of life.

Key Purpose

Coaching eligible members by making use of interpersonal communication and related techniques designed to engage clients actively and safely in health behavior change about evidence-based clinical strategies and interventions to better self-manage their health, health risks, and acute or chronic health conditions resulting in optimal wellness, improved health outcomes, lowered health risk, and decreased health care costs.

Key Outputs

Areas of responsibility may include but are not limited to:

  1. Build a strong relationship between the Health Coach and Covered Persons / Members.
  2. Create a strong sense of purpose.
  3. Analyse member's conditions, set appropriate goals for such members and create a mutually agreed upon plan focusing on key principle goals such as:
    1. Weight and diet
    2. Exercise
    3. Mental Wellbeing
    4. Condition management
    5. Custom (any goal a member may have that varies from the above categories)
  4. Provide guidance on how to create habits / sustained changes.
  5. Scenario-plan so that the member can adapt in the moment.
  6. Assess the environment in which decisions are made and look for opportunities to make healthy decisions in the path of least resistance.
  7. Achieve tangible metrics that demonstrate improvements on measurable short-and medium-term outcomes.
  8. Contact eligible covered persons / members and explain the benefits of joining a respective programme with the intention of enrolling covered persons onto the programme.
  9. Effective planning and organization of scheduled coaching interactions to optimize utilization of their time, in addition to driving adherence to their goals.
  10. Apply best industry practice coaching principles and techniques throughout a member's coaching journey which includes but are not limited to behavioural change, change management, motivational interviewing, reflective listening, self-efficacy, cognitive behavioural therapy, goal setting and overcoming barriers.
  11. Assist members with unlocking / accessing benefits available to them across the Discovery Group Platform.
  12. Assist with necessary interventions and or onward referrals in the management of complex / and or multiple conditions.
  13. Accurately capture, record and track interactions and outcomes with members throughout their coaching journey.
  14. Meet set targets as per the business objectives.
Qualifications and Experience (Essential)
  1. Clinical Qualification and registration with appropriate health professions body e.g. HPCSA, SANC, AHPCSA, SACSSP
  2. At least two years work experience in your profession
  3. Excellent communication skills, written and verbal.
  4. Proficient in MS Excel, Word, PowerPoint
Qualifications and Experience (Advantageous)
  1. Knowledge of Discovery and Vitality products, benefits, funding, and policy rules
  2. Coaching experience and or coaching qualification
