Job summary

Placements24 is recruiting a Community Health Worker to serve residents of Welkom. This field-based role focuses on health education, disease prevention, and helping community members access medical services.

You will build trusted relationships, translate health information when needed, and support individuals and families in maintaining healthy lifestyles and managing chronic conditions in the Welkom area.

Qualifications

• Minimum Grade 12 education; further training or certification in Community Health is advantageous.
• Previous experience as a Community Health Worker or in a related public health role.
• Knowledge of local health resources and challenges within the Welkom community.

Responsibilities

• Conduct health education sessions on topics such as nutrition, hygiene, and disease prevention.
• Assist community members in accessing healthcare services, including scheduling appointments and understanding health information.
• Monitor the health status of individuals within the community and report any concerns to healthcare professionals.
• Promote adherence to treatment plans for chronic diseases like diabetes and hypertension.
• Facilitate communication between healthcare providers and community members, translating medical information when necessary.
• Participate in community health screenings and outreach events in Welkom.
