Qualifications

  • Valid Code 08 driver’s license.
  • Clear criminal record.

Responsibilities

  • Supervise facility-based staff to meet all program deliverables.
  • Conduct skills audits to identify training needs.
  • Promote CCMDD external pick-up points in communities.
  • Focus on QI, Case Management, CLI, viral monitoring via eLabs, and RTCQI.
  • Collaborate with Outreach Team Lead for referral, linkage, and retention in care.
  • Monitor facility and outreach performance against TB and HIV clinical indicators.
  • Develop and implement Quality Improvement Plans (QIPs).

Job Advert Summary

TB HIV Care (THC), in existence since 1929, is an internationally recognised non-profit organisation. We provide health care services throughout South Africa to both the general population and communities at increased risk for HIV and TB, including adolescent girls and young women, sex workers, and people who use drugs. Purpose of the position :

The Facility Team Lead will oversee facility performance and ensure the successful achievement of program deliverables, including supporting QI, Case Management, Viral monitoring, CLI, eLabs, and RTCQI. They will support the Department of Health (DoH) in CLI aspects at SPI-RT Level 3 and 4 facilities, engaging at least once a month for Level 3 and quarterly for Level 4 facilities. The project aims to strengthen local capacity through technical support, mentorship, and direct service delivery to TB and HIV services, interacting with Ward Based PHC Outreach Teams (WBPHCOT) and Community Based Organisations (CBO) as needed.

Minimum Requirements

  • Bachelor’s Degree or Diploma in Nursing Sciences or health sciences
  • Diploma in Midwifery
  • Qualification in Primary Health Care (PHC) or clinical assessment (advantageous)
  • Diploma in HIV Clinical Management and/or NIMART
  • Certified training in Adult Primary Care (APC)
  • QI trained with understanding of methodology and application
  • At least 5 years relevant experience managing or implementing HAST programs with knowledge of HIV and TB treatment cascade

  • Familiarity with SAG health system at national, provincial, and district levels
  • Ability to communicate in IsiXhosa and English or other South African languages

Duties and Responsibilities

  • Supervise facility-based staff to meet all program deliverables
  • Conduct skills audits to identify training needs
  • Promote CCMDD external pick-up points in communities
  • Focus on QI, Case Management, CLI, viral monitoring via eLabs, and RTCQI
  • Collaborate with Outreach Team Lead for referral, linkage, and retention in care
  • Monitor facility and outreach performance against TB and HIV clinical indicators and advise on improvements
  • Develop and implement Quality Improvement Plans (QIPs) with the Facility Manager and team
  • Ensure staff understands and follows standard operating procedures
  • Share daily activities to ensure synergy between facility and outreach teams
  • Map disease burden and deploy outreach teams and Community Health Workers accordingly
  • Support site visits with CDC representatives and Department of Health officials
  • Support patient-tracking and referral systems implementation and monitoring
  • Mentor teams in data analytics and decision-making based on performance data
  • Cooperate with Civil Society visitors and coordinate facility and community monitoring
  • Coordinate with Facility Manager to establish and maintain facility and clinic committees
