Overview
The successful candidate will work on a one-to-one basis with patients in the comfort of their homes or caring for a patient in a facility.
Responsibilities
- Provide basic care, assisting with personal hygiene (washing, dressing, grooming, feeding, support with moving, toileting).
- Administer medications under supervision of the registered nurse or the family.
- Document the patient's condition and progress. Provide support and assistance with activities of daily living (ADLs).
- Monitor vitals and record findings.
- Perform basic wound care.
- Manage feeds of a tracheostomy patient by means of a feeding pump.
- Observe changes in patient condition and record in the daily report. Write reports in good English.
- Assess sleep routines and discuss with the registered nurse; care plans must be adjusted accordingly.
- Work as part of a multidisciplinary team (doctor, NSM or physiotherapist) to ensure tasks are completed appropriately.
- Demonstrate good communication with supervisors, colleagues, patients, and family members.
- Knowledge of dealing with emergency situations in a home setting.
- Knowledge of geriatric care, paediatric care, dementia care, post-stroke care, and ventilation care.
- Knowledge of catheter care (indwelling, supra pubic, intermittent).
- Prevention of pressure sores in bedridden patients.
- Knowledge of tracheal care or experience with tracheostomy and PEG patients.
- Knowledge of suctioning of a tracheostomy patient.
- Prevention of infection in all patients.
Skills
- A strong understanding of any treatment plans set out by other team members (Doctor, NSM or Physiotherapist) to ensure that all tasks are completed appropriately.
- Good communication skills with supervisors, colleagues, patients, and family members.
- Knowledge of dealing with emergency situations with a patient in his/her home.
- Knowledge of Geriatric Care, Paediatric Care, Dementia Care, Post Stroke Care, Ventilation Care.
- Good knowledge of catheter care (indwelling, supra pubic, intermittent).
- Knowledge of prevention of pressure sores in a bedridden patient.
- Good knowledge of trachea care or experience with unit with tracheostomy and PEG patients.
- Good knowledge of suctioning of a trachea patient.
- Infection prevention in all patients.
- Skilled in writing reports.
