Early Intervention Speech Language Pathologist

Join to apply for the Early Intervention Speech Language Pathologist role at Community Connections.

Position Details

Community Connections, Inc. is seeking a part-time licensed Speech Language Pathologist to provide SLP services for children birth – 3 in Early Intervention in La Plata, San Juan, and/or Archuleta counties.

Flexible schedule with hours ranging from 2-16 hours per week.

Services are provided in homes and community settings, both in-person and via Zoom/telehealth.

We promote a coaching model to empower parents and teachers to implement strategies between visits.

Qualified applicants should enjoy working with young children and supporting families as part of a team.

Spanish language skills are ideal but not required. Knowledge of Indigenous (Diné and/or Ute) or Hispanic cultures is also a plus but not necessary.

Compensation is $35-40/hr DOE, including mileage, covering direct services, preparation, and travel time.

Responsibilities
  • Participate in insurance billing and become a Medicaid provider.
  • Assist in developing and implementing Individual Family Support Plans (IFSP).
  • Collaborate with Service Coordinators to identify family priorities and gather resources.
  • Attend IFSP meetings, reviews, and participate in trans-disciplinary teams.
  • Conduct ongoing assessments with family input and maintain accurate documentation.
  • Stay current with trends and best practices in Early Intervention.
  • Respond professionally to families' needs and communicate effectively with team members.
Qualifications
  • B.A. or Master's in a relevant field (e.g., Speech-Language Pathology) and licensure for Colorado.
  • Experience with children birth-3 preferred.
  • Must possess a valid driver's license and auto insurance, and pass background checks.
Physical Demands

Tasks may involve computer work, community travel, and in-person interactions. Attendance at meetings, training, and community activities is required.
