Eunoia Lane is a locally owned and operated company based in Darwin, providing services to NDIS participants across the Northern Territory, including Coordination of Supports, Specialist Support Coordination, and Occupational Therapy. We are passionate, enthusiastic, and committed, and are seeking a proactive and motivated Support Coordinator to join our team.

About the position

The Support Coordinator's role involves coordinating supports for NDIS clients and assisting them in achieving their goals outlined in their NDIS plans. This includes proactively identifying, developing, coordinating, and facilitating access to supports and services alongside clients.

The ideal candidate will be highly organized, with excellent oral, written, and interpersonal communication skills. They should work well independently and as part of a team, demonstrating self-discipline and effective time management. Analytical and creative skills are essential to assess individual needs and report on outcomes. A positive attitude towards people with disabilities and the ability to engage effectively with diverse cultures are also required.

Key responsibilities
  1. Work with clients, families, and carers to identify opportunities for skills development, education, and employment.
  2. Support and encourage clients to achieve their goals, providing support coordination in line with their NDIS plans.
  3. Develop networks and collaborate with stakeholders to identify, plan, implement, and evaluate supports.
  4. Maintain accurate case management notes and reports on client progress.
  5. Liaise with community services, government agencies, customers, and service providers.
  6. Participate in team meetings, discussions, and planning sessions.
  7. Generate ideas and options, maintaining flexibility and openness to change.
Qualifications and requirements
  • Current driver's license and access to a roadworthy vehicle.
  • Working with Children (Ochre) Card.
  • National Police Clearance.
  • First Aid Certificate.
  • NDIS Worker Screening.
  • Right to work in Australia.
