Disease and Care Coordinator (DRM Case Manager) (Pretoria)

Join a dynamic team dedicated to improving patient outcomes! Our client seeks to employ a Disease and Care Coordinator to provide active disease management clinical care support and health education to medical scheme beneficiaries with chronic conditions. This role integrates clinical care and self-management strategies to enhance patient wellbeing.

Key Responsibilities :

1. Conduct comprehensive patient assessments and develop personalised care plans
2. Provide education and coaching on chronic disease management
3. Monitor patient progress and liaise with healthcare providers for optimal care
4. Support case management and preauthorization teams with patient interventions
5. Maintain accurate records and contribute to case studies

Requirements :

• Registered Nurse (Postgrad in Counselling / Psychology beneficial) OR Clinical Associate (BCMP)
• SANC / HPCSA registration
• Minimum 3 years experience in Disease Management / Care Coordination within a Managed Care setting
• Expertise in Diabetes Disease Management (Maternity Wellness experience is a plus!)
• Strong patient engagement, communication, and organisational skills
• Proficiency in MS Office; multilingual abilities advantageous
