Support Worker Job Description

About Us: Lifestyle Solutions provides innovative and flexible person-centered support for people living with a disability. We assist customers to become more independent in their own homes and communities.

Responsibilities include:

  • Mentoring and skill development, guiding customers to explore and develop their personal interests and hobbies.
  • Coaching on independent living skills such as cooking, domestic, and social skills.
  • Administering medication and adhering to individual support plans.
  • Completing clear, concise, and factual handover notes and incident reports.
  • Providing personal care, including toileting, showering, feeding, and dressing when required.
  • Helping customers engage with their local communities through Community Participation Programs and recreational activities.
  • Personal care experience is essential; must be comfortable and knowledgeable in providing it.
  • Knowledge or training in diabetes management and PEG feeding is advantageous.
  • Community access may be required at times.

What you will bring:

  • Willingness to work a variety of shifts, including mornings, afternoons, evenings, sleepovers, and weekends.
  • Versatile communication skills, including interacting with both verbal and non-verbal customers.
  • Current NDIS Worker Screening Clearance (or willingness to obtain).
  • Current First Aid Certificate (HLTAID003).
  • Formal qualifications in Disability Community Services or similar are advantageous.
