Placements24 is seeking a motivated Community Health Worker to serve residents of Nelspruit and surrounding areas. This frontline role focuses on health education, preventative practices, and connecting people with essential services in a collaborative healthcare setting.

You will conduct outreach, perform basic screenings, support patients with chronic conditions, collect health data, and travel within the community.

Qualifications

• Certificate or Diploma in Community Health Work or related field.
• Previous experience as a Community Health Worker or in a similar role.
• Knowledge of local health resources and services in the Nelspruit area.
• Strong communication and interpersonal skills.
• Ability to work effectively with diverse populations and travel within the community.
