Job summary

Placements24 is seeking a Community Health Program Coordinator in Pretoria to connect community members with health services and promote preventative care. You will collaborate with clinics, leaders and underserved groups to ensure equitable access and improved health outcomes.

The role requires a Diploma/Degree in Nursing, Public Health, or Social Work, at least 3 years of relevant experience, and strong communication abilities.

Qualifications

• Diploma or degree in Nursing, Public Health, Social Work, or a related field.
• Proven experience in community health program coordination or similar roles.
• Strong communication, interpersonal and cross-cultural skills.
• Experience developing and delivering health education programs.
• Proficiency in data collection and report writing.

Responsibilities

• Coordinate community health programs, including health education, screenings, and outreach.
• Liaise with healthcare providers, community organizations, and local government in Pretoria.
• Develop health education materials for diverse community groups.
• Organize health awareness campaigns and workshops.
• Facilitate access to healthcare services for underserved populations; assist with appointments and referrals.
• Collect data on participation and health outcomes; prepare regular reports.
• Manage program logistics, scheduling, resources and supply management.
• Provide support and guidance to community health volunteers.
