Placements24 seeks a proactive Community Health Facilitator to lead outreach programs and promote health education in Centurion, Gauteng. You will engage underserved communities to improve access to services and health literacy.

You will plan, implement, and evaluate health initiatives, working with community leaders and local organizations to measure impact and drive continuous improvement.

Qualifications

• A qualification in Public Health, Community Development, Health Sciences, or a related field.
• Proven experience in community engagement and health program facilitation.
• Strong understanding of public health principles and challenges in South Africa.
• Excellent presentation, facilitation, and interpersonal skills.
• Ability to work independently and manage multiple projects.

Responsabilités

• Design and deliver health education workshops and awareness campaigns in community settings.
• Identify community health needs through surveys, focus groups, and stakeholder consultations.
• Facilitate access to primary healthcare services for community members.
• Develop culturally appropriate health materials and resources.
• Build and maintain strong relationships with community leaders, CBOs, and local government.
• Monitor and evaluate the effectiveness of outreach programs, providing regular reports.
• Collaborate with healthcare professionals to ensure integrated health support.
• Promote healthy lifestyles and preventative health practices within the target communities.
• Manage program budgets and ensure efficient resource allocation in Centurion, Gauteng.
• Represent the organization at community events and health forums.
