The successful candidate will be responsible for ensuring that the quality pharmaceutical services, financial objectives, legislative compliance and people management requirements is achieved to the required standards.

Function Pharmacy Facility Life Peglerae Hospital Position Clinical Practice Pharmacist Introduction

A vacancy exists for a Clinical Practice Pharmacist based at Life Peglerae Hospital, reporting to the Pharmacy Manager, Sonique van den Berg. The successful candidate will be responsible for ensuring that the quality pharmaceutical services, financial objectives, legislative compliance and people management requirements is achieved to the required standards.

Critical Outputs
  • Perform daily ward rounds with a multitude of medical specialists
  • Coordination of antimicrobial stewardship initiatives, identification of problem areas and implementation of corrective actions
  • Active participation within the hospital’s clinical pharmacy team and clinical mentoring of pharmacists and pharmacist interns
  • Review prescriptions and monitor patients to ensure effective and safe drug therapy, provide feedback and advice to the healthcare team regarding potential problems
  • Collaborate with Antimicrobial Stewardship Champion Doctor, to identify and address inappropriate antimicrobial usage.
  • Perform pharmacokinetic calculations and interpretation when required
  • Participate in the implementation of and compliance to LHC Quality Management Systems.
  • Practice of evidence-based patient care
  • Identifying and disseminating clinical evidence to support clinical pharmacy and antimicrobial stewardship interventions, ensuring cost-effective management of patient care
  • Participate in the active management of cost of sales of pharmaceuticals in line with revenue and budget.
  • Build effective working relationships with doctors, allied healthcare professionals, infection prevention and nursing staff
  • Drive the delivery of Key Performance Indicators in clinical pharmacy processes and practice, to improve patient safety and outcome
  • Preparation for and active participation in internal stakeholder meetings, including presentation of progress and gaps
Requirements
  • Pharm is essential, a qualification in a clinical pharmacy skills program would be advantageous
  • A minimum of 2 years hospital experience required
  • Relevant clinical knowledge and experience in the critical outputs
  • Experience within an antimicrobial stewardship team, implementation of antimicrobial stewardship and clinical pharmacy initiatives would be advantageous
  • Current registration with South African Pharmacy Council as a pharmacist
  • Understanding of pharmacy and related healthcare industry, legislation, regulation and challenges
