This is a Permanent/Fixed Term Full Time appointment at 80 hours per fortnight, with the possibility of extension and/or permanency.

This position is based at Rockingham Kwinana Mental Health Service.

Position Description: PaRK Mental Health Service is offering an exciting new opportunity for a highly skilled and motivated Clinical Nurse, Mental Health to collaborate alongside the Clinical Nurse Specialist in the Wellness Clinic. The Wellness Clinic will focus on improving the physical health outcomes of clients living with enduring mental health disorders. Clinicians from the Clinical Treatment Team (CTT) case manage suitably identified clients with the principal goals of providing monitoring and the management of side effects and adverse effects of psychotropic medications, Metabolic monitoring, wellness promotion, and where appropriate the supported transition to shared care and primary care arrangements. The model of care will be both a co-managed and transitional model. The co-management refers to client care being coordinated by both the Community Treatment Team (CTT) medical case manager or nursing and allied health case manager and the Wellness Clinic nurses. The transitional aspect will have a focus on engagement with primary care/GP to work towards a period of shared care with the GP. The successful applicant will:

  • Be passionate about improving the physical health of people with serious mental illness and have knowledge of standards of care in relation to the physical care of mental health consumers.
  • Have demonstrated knowledge in the care and management of clients treated with long-acting injectable medication and other psychotropic medications.
  • Have a strong focus on working collaboratively with the Wellness CNS and GP Liaison CNS positions and other key stakeholders to transition suitable clients to shared care or primary care arrangements.
  • Assist the clinic in the provision of care attending the clinic for the purpose of LAI administration.
  • Will act as a resource for the wider Mental health team and assist in delivering education and training as required.
