North Bay Regional Center (NBRC) is seeking a Social Services Case Manager (Service Coordinator) to oversee services for individuals with developmental disabilities in California. The role emphasizes coordinating resources, monitoring plans and ensuring client needs are met with a person-centered approach.

The position requires a Bachelor’s degree in a related field; a Master’s degree is preferred and at least 3 years of relevant experience.

Competenze

• Bachelor’s Degree in a field related to developmental disabilities (e.g. Social Work, Psychology, Counseling, Special Education, and/or Nursing)
• Master’s Degree preferred
• 3 years’ experience working with individuals with developmental disability
• Bilingual language ability (Spanish to English) preferred

Mansioni

• Manage delivery of services to NBRC clients as per Individual Program Plan
• Use person-centered planning to assist clients in accessing resources before using NBRC funds
• Monitor vendor services and verify services align with purchase of service authorizations
• Conduct annual reassessments of clients’ developmental status with the interdisciplinary team
• Conduct client assessments on an occasional basis

Conoscenze

Bilingual (Spanish→English)

Formazione

Bachelor’s Degree in Social Work, Psychology, Counseling, or related field

Master’s Degree preferred
