North Bay Regional Center is seeking a Social Services Case Manager to coordinate services for individuals with developmental disabilities. You will manage service delivery, support clients in pursuing Individual Program Plans, and work closely with an interdisciplinary team.

Qualifications include a related Bachelor’s degree and 3 years of experience; bilingual Spanish/English is required. Remote work is not available; location within commuting distance to NBRC offices is expected.

Formación

• Bachelor’s Degree in field related to developmental disabilities (as stated).
• Master’s Degree preferred.
• Bilingual Spanish/English required (as stated).

Responsabilidades

• Use person-centered planning to assist clients in utilizing resources.
• Monitor vendor services and verify delivery as per service authorizations.
• Evaluate clients’ Individual Program Plans and ensure community resources are available.
• Conduct annual reassessments and occasional client assessments.

Conocimientos

Bilingual Spanish/English

Educación

Bachelor’s Degree in relevant field (Social Work, Psychology, Counseling, Special Education, Nursing)

Master’s Degree preferred
