The role requires a Bachelor’s degree in Psychology or Social Work, CPR certification, English proficiency, and strong communication and documentation skills to support coordinated care and smooth transitions.

Formación

• Bachelor’s degree in Psychology, Social Work, or closely related field.
• One year of behavioral health experience preferred.
• English proficiency required.

Responsabilidades

• Provides case management and discharge planning to patients and families.
• Coordinates follow-up appointments with psychiatrists.
• Documents discussions and maintains EMR records.
• Maintains professional relationships with internal and external clients.
