Job summary

The City of Rock Hill is seeking a dedicated Case Manager for YouthBuild, onsite to support participants from enrollment through follow-up. You will conduct intake, assess needs, and create Individual Service Strategies aligned with educational, vocational, and personal development goals.

Responsibilities include counseling, coordinating referrals to housing, transportation, mental health, and legal services, tracking progress in data systems, and following up for 12 months post-exit to document

Qualifications

  • Bachelor's degree as specified; related field considered.
  • Minimum of two years of experience in case management, counseling, or youth development.
  • Bilingual ability preferred; experience with diverse populations.

Responsibilities

  • Conduct intake and comprehensive assessment to determine eligibility for the YouthBuild program.
  • Develop, implement, and update Individual Service Strategies (ISS) reflecting education, training, employment and personal development goals.
  • Provide ongoing individual and group counseling addressing barriers such as housing, transportation, childcare, mental health, substance use, legal issues, and family conflict.
  • Coordinate referrals to supportive services and community partners, including behavioral health providers, housing programs, and legal aid.
  • Monitor attendance, academic progress, and behavior; implement retention strategies to reduce attrition.
  • Enter and maintain data in federal reporting/data systems to support PIRL reporting and performance tracking.
  • Follow up with participants for at least twelve months after program exit to document outcomes.

Job description

General Description:The purpose of this class within the organization is to provide individualized support, advocacy, and service coordination to YouthBuild participants throughout their enrollment and follow-up period. The Case Manager conducts comprehensive assessments, develops Individual Service Strategies (ISS), connects participants with supportive services, and monitors participant progress toward educational, occupational skills, credential attainment, and employment goals in accordance with DOL/ETA program requirements and performance measures. This class works under general supervision, independently developing work methods and sequences.Minimum Education and Experience Requirements:Bachelor's degree in social work, counseling, human services, education, or a related field (relevant experience may substitute for degree on a year-for-year basis). Minimum of two (2) years of experience providing case management, counseling, or youth development services, preferably to opportunity youth or justice-involved young adults.The equivalent combination of education and related work experience may be considered.Preferred Qualifications:Master's degree in social work or counseling. Prior experience with WIOA Youth, YouthBuild, Job Corps, or other DOL-funded youth programs. Bilingual or multilingual ability relevant to the population served. Lived experience or strong cultural competency working with justice-involved or disconnected youth.Special Certifications and Licenses:Valid Driver’s LicenseDesirable Knowledge, Skills and Abilities:Knowledge of youth development principles, motivational interviewing, and trauma-informed care practices.Experience maintaining case records and entering data into case management or federal reporting systems.Strong interpersonal, organizational, and crisis-response skills.Proficiency with data management systems and Microsoft Office Suite.Ability to plan, organize and prioritize daily assignments and work activities.Ability to communicate clearly and concisely, verbally and in writing, and to listen effectively.Ability to operate modern office equipment including computers, copiers, cell phones, and tablets in the performance of daily activities.Ability to make sound, educated decisions with minimal supervision.Ability to manage heavy workload by planning, organizing and setting priorities in order to meet strict deadlines.Ability to deal with people beyond giving and receiving instructions. Must be adaptable to performing under stress when confronted with emergency situations or tight deadlines.Ability to maintain positive customer-focused relationships with co-workers, supervisors, agencies, the general public, and all other internal and external customers. Personally demonstrates appropriate customer service skills.Work is performed onsite. The incumbent must be present at work to perform the essential functions of this job.Essential Job Functions:Conduct intake, orientation, and comprehensive assessment of prospective participants to determine eligibility and suitability for the YouthBuild program.Develop, implement, and regularly update Individual Service Strategies (ISS) in partnership with each participant, reflecting education, training, employment, and personal development goals.Provide ongoing individual and group counseling addressing barriers to success such as housing, transportation, childcare, mental health, substance use, legal issues, and family conflict.Coordinate referrals to supportive services and community-based partners, including behavioral health providers, housing assistance programs, and legal aid.Monitor participant attendance, academic progress, and behavior; implement retention strategies to reduce attrition.Maintain accurate, timely, and confidential case files consistent with grant recordkeeping and privacy requirements.Enter and maintain participant data in the required federal reporting/data system to support PIRL reporting and performance tracking.Support the leadership development and community service components of the program, including mentoring, service-learning projects, and youth policy committee activities.Coordinate closely with the Job Developer and instructors to align case plans with occupational skills training and job placement goals.Conduct structured follow-up with participants for a minimum of twelve (12) months after program exit to document education, employment, and credential outcomes as required by the grant.Assist participants with career exploration, goal setting, and connections to postsecondary education opportunities.Participate in staff meetings, case conferencing, and professional development/training as required.Performs all other duties as may be assigned to meet organizational needs.This Notice is not intended to be an all-inclusive list of duties, knowledge, skills and abilities required to do the job.
