Job summary

Eckerd Connects seeks an After Care Resource Navigator in Miami, FL. The role supports reentry for youth by coordinating resources and transition planning with families, JPO, and service providers. You will assess service needs, facilitate referrals, and initiate support services to promote successful community reintegration.

The position requires a degree or equivalent experience, a valid driver's license, and a clean background. Competitive benefits and a $22.20 hourly rate are offered.

Qualifications

• Bachelor's degree or associate degree with two years of adolescent work experience.
• 4+ years of experience with adolescents may substitute the education requirement.
• Must pass a drug screen per policy.
• Must have a vehicle and be able to provide annual auto insurance; driving for business.
• Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse.

Responsibilities

• Actively participate in the Transition Conference and related meetings; assist with Transition Plan for youth.
• Meet with Parent/Guardian prior to release to establish family relationships and needs.
• Identify resources with JPO to address youth and family needs.
• Coordinate referral process for identified resources with youth, family, and JPO.
• Initiate services for identified resources for youth and family.
• Maintain contact with youth, family, and resources to monitor progress and address roadblocks.
• Establish relationships with DJJ staff, law enforcement, judges, and attorneys.
• Maintain up-to-date youth case files and notes.
