Job summary

State of South Carolina's Department of Social Services seeks a Bridge Navigator Coordinator to support current and former foster youth through the Extension of Foster Care program. You will partner with county staff to ensure access to assessment tools and a strength-based plan aligned with youth development, trauma-informed approaches, and continuity of care across life domains.

Qualifications

• Master's Degree and one (1) year of professional experience in social services programs, correctional, education, business administration, general administrative management, or relevant program experience.
• Bachelor's Degree and three (3) years of professional experience in social services programs, correctional, education, business administration, general administrative management, or relevant program experience.

Responsibilities

• Bridge Navigator Coordinator provides transitional support for current and former foster youth in the Extension of Foster Care (EFC) program.
• Collaborate with county foster care staff and regional TSS division staff to ensure access to assessment tools and plan development.
• Attend Transition Plan Meetings and support facilitation, promote youth voice, and connect youth to benefits and services.
• Complete and maintain certification to conduct EFC Young Adult Transitional Teaming (YATT) reviews and participate in case reviews.
