Southwest Key Programs is seeking a Youth Tracker to perform daily face-to-face contacts with youth, working across multiple sites in Conyers and Athens. The role requires flexibility to work evenings, weekends, or holidays as needed.
The Youth Tracker will support youth under service plans, maintain accurate records, and enter data into the JTS and ETO systems, with a focus on safety and accountability. A valid driver’s license and background check are required.

The Youth Tracker is responsible for completing assigned daily contact with youth, as well as meeting the conditions of the youth’s service plan for assigned caseload. The Youth Tracker must maintain a flexible work schedule that may include working hours other than regular work schedule and evenings, weekends and holidays.
Must be able to obtain First AId Certification (CPR). Must be able to verify client’s whereabouts by driving, sitting, standing and walking for extended periods of time. Daily use of various office equipment. Bending, stooping, climbing up stairs and lifting up to 15 lbs. required to complete daily tasks.
This is a youth tracking program which requires daily contact at various client locations. Work shifts are subject to change. Noise levels vary from mild to moderate. This position may require extended periods of time in an automobile. Exposed to prevalent weather conditions. This position will require 85% travel to various GA counties (max up to 1 ½ hour one way), and 15% in the office. Staff are required to use their own vehicle for tracking. Travel reimbursement (IRS Mileage Rate) is provided to staff.
