Job summary

HELP of Southern Nevada is seeking a dedicated Case Manager for our Emergency Shelter Program in Las Vegas. Under supervision, you will provide direct case management services to homeless and at-risk youth, developing client-centered plans and coordinating services to address needs.

You will facilitate routine sessions, document progress in HMIS, and work with diverse populations, ensuring safety, confidentiality, and timely completion of reports and exit summaries.

Qualifications

• Bachelor Degree in Social Work or 4 years of experience equivalent to a bachelor degree.
• Professional experience in case management or related social services.
• Must pass TB test and have CPR certification, kept current.

Responsibilities

• Create Client Centered Case Plan within 72 hours of intake.
• Monitor progress and facilitate weekly sessions.
• Enter case notes daily into HMIS reflecting progress.
• Maintain audit-ready files and complete exit summaries within 72 hours of exit.
• Organize and facilitate daily groups and supervise residents during shifts.
• Attend supervision meetings and report program violations promptly.
