Job summary

Goddard Riverside's Stanley M. Isaacs Neighborhood Center in New York, NY seeks an experienced, dynamic case manager for the Education and Workforce Program, serving ages 17–24.

You will carry a caseload of up to 30 participants and work with a multidisciplinary team to address barriers, set SMART goals, and support educational and employment objectives. Qualifications

• 1–2 years of human services, education, community development, social work, or relevant experience.
• Bachelor's degree in human services field.
• Strong writing, speaking, interpersonal and organizational skills.
• Ability to engage and work with a diverse population and community resources.

Responsibilities

• Complete biopsychosocial assessments and identify SMART goals for caseload participants.
• Provide weekly or bi-weekly sessions for all participants on caseload.
• Maintain case notes and treatment plans per funder and agency policies.
• Identify barriers to self-sufficiency (economic, education, employment).
• Help participants achieve educational and workforce goals and access services.
• Screen for benefits and refer to community resources as needed.
• Provide individualized case management and navigate complex systems (healthcare, housing, etc.).
• Facilitate weekly social-emotional skills workshops.
• Track referrals and ensure timely documentation and reporting.
