Job summary

VTSS in Vermont seeks a full-time Permanency Coordinator to support at-risk youth and families across Barre, Hartford and surrounding areas. The role emphasizes short-term stabilization, discharge planning, and coordination of community resources; travel is expected weekly with a company vehicle provided.

The right candidate has post-training therapy experience, licensure eligibility, and experience with traumatized children/adolescents.

Qualifications

• Post-training therapy experience is required.
• Eligibility for licensure is required.
• Background working with traumatized children/adolescents.

Responsibilities

• Travel an average of 8+ hours weekly throughout Vermont to families' homes.
• Coordinate treatment and discharge planning.
• Manage documentation and participate in weekly team meetings.
• Support, educate, and guide families in developing structure and healthy routines.
• Engage with community services, schools, and agencies.
