Job summary

The Key Program, Inc. in Springfield, MA is seeking a Youth Case Manager (Outreach Caseworker) to work with children, youth and families in Hampden County. This full-time role requires evenings, weekends and holidays, with on-call duties.

You will oversee services for clients referred by contracting agencies and collaborate with the regional treatment team. Qualifications include a bachelor’s degree in a human service-related field and at least one year of direct care experience, plus valid

Bachelor’s degree in a human service-related field is required.
21 years of age or older; valid state driver’s license; insured car for work use.
Written and verbal communication skills; ability to work independently and on a team.

Responsibilities

Develop and maintain supportive client relationships with accountability and treatment goals.
Provide advocacy in health, education, vocation, legal and family/foster care areas.
Facilitate individual and group counseling, mediation, and support sessions.
Coordinate clinical services with clinic staff.
Locate and utilize community resources for clients and families.
Complete case documentation thoroughly and timely.
