Job summary

Northwestern Community Services Board is seeking a dedicated clinician to manage an assigned caseload of children and adolescents with mental health needs.

You will assess client needs, develop and review service plans, and coordinate with community resources to meet stated goals and objectives.

Required qualifications include a Bachelor's degree in a relevant human services field, QMHP eligibility, and at least one year of experience working with children; a valid driver’s license is also needed.

Qualifications

  • Bachelor’s degree earned from an accredited college or university in relevant human services field required.
  • At least one year of experience working with children required.
  • Qualified Mental Health Professional (QMHP) eligibility required.
  • Must possess a valid Driver’s License, good driving record, and the ability to provide your own transportation.

Responsibilities

  • Participates in screening of new service requests within CYFS by approved procedure and ensures adequate services.
  • Regular interdisciplinary staff meetings are held at the position location.
  • Accepts newly assigned cases after diagnostic intake and completes case management assessment.
  • Develops treatment/service plans and works with clients and families to implement services.
  • Provides ongoing supportive and/or case management to help clients achieve goals.
  • Conducts regular face-to-face reviews to monitor progress and modify plans as needed.
  • Documents all service contacts and maintains case records per standards.
  • Participates in interagency planning to improve service continuity and effectiveness.
  • Meets with supervisor for professional growth and administrative issues.
  • Maintains communication with the consulting psychiatrist regarding medication and adjustments.
  • Maintains assigned caseload and performs other duties as assigned by the supervisor.
