Job summary

Clarvida is seeking an Independent Living Coordinator to support youth and families in Juniata County, PA. This community-based role focuses on promoting independent living skills, goal attainment, and successful transitions into adulthood, with strong emphasis on relationship-building, organization, and documentation.

Qualifications

• Bachelor’s degree in related field required.
• Bachelor’s degree in an unrelated field with at least 12 credits in Human Services required.
• Associate’s degree with 2 years’ experience with children, youth, and families required.

Responsibilities

• Initiate and maintain contact with youth referred through county services.
• Develop Independent Living Goal Plans within the first month of service.
• Complete quarterly reviews of Independent Living Plans with participating youth.
• Schedule, plan, and facilitate group activities based on youth needs.
• Collaborate with families, county agencies, and community partners to support service goals.
• Provide guidance, support, and resource coordination to youth and families.
• Prepare and maintain accurate, timely, and compliant documentation, including case notes.
• Ensure compliance with agency policies, state regulations, and program requirements.
• Attend meetings, case reviews, and court proceedings as needed.
• Provide professional court testimony when required.
