Intercept Health is hiring a Life Skills Coach/Case Manager for the Young Adult Independent Living Program in Norfolk, VA. This full-time role offers a $2,000 sign-on bonus and an hourly rate of $21–$23, with responsibilities focused on guiding residents through transition to independent living.

You will help residents develop money management, employment, education, and daily living skills, while supporting plan development and providing transportation and administrative support.

Competenze

• Bachelor's degree in any field with two years of case management experience for children and families.
• Human services degree with related case management experience preferred.
• Master's degree in a human services field with relevant placement experience.

Mansioni

• Engage with young adults (ages 17-21) in the Independent Living Apartment Program.
• Model and teach money management, employment, education, daily living skills, and career development.
• Contribute to resident transition plans and assist with life skills, transportation, and administrative duties.

As a Full-time Life Skills Coach at YouthQuest's Independent Living Apartment Program, you will actively engage with young adults, ages 17-21, who are preparing for successful transition into the community. You will serve as a positive role model for residents to observe and learn socially effective values, attitudes, and behaviors. You will support residents in developing money management skills, obtaining employment, completing their education as applicable, learn daily living skills, and explore career development opportunities. You will contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties.

Education and Experience Required

The selected candidate will have a Bachelor's diploma in any field plus two years of experience in providing case management services to children and families; OR a human service Bachelor's degree (Social Work, Psychology, Counseling, Special Education, etc) with experience providing case management services to children/youth and their families preferred; OR a human service Master's degree (Social Work, Psychology, Counseling, Special Education, etc) with a student placement providing case management services to children/youth and their families.
