Job summary

The Link Minnesota is seeking a dedicated Case Manager to work with Lindquist Apartments residents, assisting youth and families in achieving housing stability and self-sufficiency. The role emphasizes harm-reduction and youth-centered approaches within a multi-service nonprofit setting.

Position requires driving, flexible schedule, and the ability to navigate complex social systems to connect clients with health, education, and housing resources.

Qualifications

• Desire to support youth and families affected by poverty and social injustice.
• Experience delivering case management and supportive services to at-risk populations.
• Strong communication, organization, and documentation skills.
• Ability to work with diverse populations and maintain confidentiality.

Responsibilities

• Provides case management services focused on housing stability for Lindquist Apartments residents.
• Identifies needs related to health, wellness, and education and connects to resources.
• Conducts assessments to determine barriers and resource needs for planning.
• Acts as advocate/liaison between clients, landlords, and service providers.
• Maintains client records and reports progress to supervisors and funders.
• Provides transportation as needed to meet client needs.
