• Collaborate with Day Treatment staff, school personnel, families, DSS, MCOs, and community providers to coordinate care.
• Attend Child and Family Team meetings and other treatment planning meetings as needed.
• Assist with treatment planning, placement coordination, and outside referrals.
• Complete timely and accurate clinical documentation, including therapy notes, assessments, treatment plans, and clinical supervision documentation within the electronic health record (EHR).
• Provide crisis intervention and de-escalation support when necessary.
• Maintain compliance with agency, state, and Medicaid documentation requirements.
• Perform other duties as assigned.
